Urinoma

The case involves a 77 year-old man with a history of severe benign prostatic hypertrophy. He had been advised to have a TURP in the past but had repeatedly declined. He was hospitalized for nausea, vomiting and R flank pain without hematuria occurring over a week's time, and it was noted that his creatinine had elevated from its baseline of 1.3 to 7. A CT scan on arrival showed the following.

This patient had developed a R-sided urinoma, which is simply a collection of urine which results from rupture of the collecting system due to high pressures from obstruction. It is more typically seen in renal transplant patients post-operatively at the site of the surgical anastamosis between the recipient and donor collecting systems; non-surgical cases of urinomas such as this one are more rare. The patient had a decompressive Foley catheter placed and an IR drain placed to remove the fluid collection. The fluid's creatinine level was higher than serum level, indicating urine as the source. He experienced a post-obstructive diuresis with a lowering of his creatinine following these interventions.

A technique for treatment of patients with cystic involvement of femoral neck-trochanteric part

In Russian Ilizarov Scientific Center "Restorative Traumotology and Orthopaedics" the rational technique for reconstructive surdies on the basis of transosseous osteosynthesis was worked out. This method allows not only to get rid of destruction focus at a single stage, but to eliminate the existed associated orthopaedic pathology as well.

The indication to operative intervention is:
- Absence of positive dynamics reparative processes in the pathological center at conservative treatment;
- Dynamic increase in the sizes of the center of destruction;
- Numerous pathological crises or threat of their occurrence;
- Availability of deformations of a segment with shortening or without it;
- Relapses after earlier executed operative interventions.

The given way is applied at 7 patients with cystophorous defeat of a proximal area of a femur in the age of from 8 till 16 years (men – 5, women – 2).
Earlier at patients 11 pathological crises in occasion of which it was applied conservative (a plaster bandage - 5, skeletal extension - 2) and operative (a regional resection - 3, an intraperiosteal resection - 1) treatment were marked.

At receipt in a hospital at five patients it was marked coxa vara from 1200 up to 850, at 2 - a plough valga (1350 - 1450). In two cases there was a shortening a segment on 3 and 4 sm accordingly. The casualty finiteness was 3 cm longer (due to a hip) than contralateral at two patients.
The size of distribution of pathological process on bones was various and made from 4 up to 10 cm.

Terms of an osteosynthesis the device have made 106,7±14 days. Favorable outcomes of treatment are reached in all cases. Results were estimated in a view of following factors:
- Presence of a painful syndrome;
- Stability in a place of operative intervention;
- Presence of deformation, shortening of a segment;
- Volume of movements in adjacent joints;
- Occurrence of relapse of disease.

The remote results of treatment are studied in terms from 1 year till 5 years.
Thus, use of the developed method on the basis of perosseous an osteosynthesis at treatment of patients with cystophorous defeat of a femur, providing performance directed osteotomy, receptions of a compression and distraction fragments as separately, and in their combination, allows to receive stable, recurrence-free results with adequate orthopedic and functional rehabilitation and to reduce term single-stage treatments in 1,5-2 times.
